Long-Term Mental Health Care Coordination encompasses the
following case management-related goals:

Long-Term Care Coordination activities include:

including payee services if needed
Housing in the Community

AMHP
also provides
short-term care coordination for persons moving from
hospital or private pay treatment to the public system.

AMHP
assists seniors to live successfully in the familiar
settings of their own homes, assisted living situations, and
in other facilities. Our mental health expertise enhances
our services to adults with long-term mental health problems,
and/or dementias, including Alzheimers disease.
We provide the following
