Geriatric Care

AMHP assists seniors to live successfully in the familiar settings of their own homes, assisted living situations and other facilities. Our mental health expertise enhances our services to adults with long-term mental health problems, and/or dementias including Alzheimer’s disease.

Specialized Services

We provide the following services for geriatric care:

• Assessment and evaluation of current care and future options
• Guidance and consultation services for clients, families, and caregivers
• 24-hour emergency telephone service
• Coordination of care with other professionals
• Mental health treatment
• Supportive counseling for a wide variety of issues, including terminal illnesses
• Payee services
• Monitoring of in-home care
• Housing placement
