Services For Those With Medical Issues

AMHP provides intensive case management services for people with primary medical diagnoses who also have mental health issues which complicate treatment. We help physicians as well as patients through:

  • Coordinating care with other professionals involved with the patient
  • Assisting clients in monitoring medications
  • Assisting with following through on doctor's instructions
  • Providing objective information to the physician by being in contact with patients between appointments
  • Working with client life style changes necessary to address medical issues
  • Working with mental health issues including depression, grief, psychotic symptoms, or difficult behaviors
  • Transport to appointments
  • Assisting with maintaining and improving quality of life
